This is the first many resignations that are expected to be tendered as investigations gather steam.
It was confirmed by Premier Stan Mathabatha, who added that he had accepted the resignation.
He stated that Mhlongo had resigned because kid personal reasons which he did not make public.
Mhlongo, together with the chief financial officer Justice Modau and other senior officials, was undergoing an internal disciplinary process for their role in the PPE scandal.
This stems from a scandalous R125 million PPE tender.
In the report the SIU found, among others, that the provincial health department irregularly awarded PPE tenders worth R125 million.
Many irregularities about the awarding of the tender were noticed during internal investigations.
Many prominent politicians have either been fired or they have resigned over PPE tenders.
